Diana Duclos-King, MN, RN (EC)
Diana is a recent addition to the CKLFHT in Bobcaygeon, taking over Linda Ready’s practice. Originally from Quebec, she has a DEC (Diplôme d’études collégiales) in Nursing from Dawson College, a BScN from St. Francis Xavier University and a Master of Nursing in Nurse Practitioner from Athabasca University. Her nursing background is emergency/critical care. Diana has a proactive and hands-on approach to patient-centered care and looks forward to getting to know the area.
Alicia Welby, Medical Receptionist
Alicia joined our team in May of 2022 and works alongside Diana in the Bobcaygeon office. She recently graduated Centennial College completing her Office Administration – Health Services Diploma with honours. This included learning medical transcription, medical terminology, medical office procedures, and medical billing. Previously Alicia has worked retail gaining a tremendous amount of interpersonal skills. Alicia grew up in Oshawa and now lives in Omemee.

Sirui Zhang, Nurse Practitioner
Sirui has a Masters of Nursing from Queen’s University (graduated July 2023), and a Bachelor of Nursing from University of New Brunswick, where she was a recipient of a scholarship and recognized on the Dean’s List. She also has a Bachelor of Clinical Medicine from Harbin Medical University in China. As part of her NP program, Sirui had her placements at St. Lawrence District Medical Centre, Upper Canada FHT, and Maple FHT. She has also worked as an RN in Neurology at Kingston General, and OB/GYN at Saint John Regional Hospital.
Kylie Pankhurst, B.Sc.N., RN
Kylie works as a Registered Nurse, as well as the team’s Professional Practice and Education Lead, and has been with the Family Health Team since its beginning in 2011. She holds a Bachelor of Arts in Community Health from Brock University as well as a Bachelor of Science in Nursing from Trent University. Kylie lives in Bobcaygeon with her husband and two daughters.

Has 20 years RN experience in surgical, OR and private in-home nursing.
Comes to us with 10 + years as office administrator with local retired MD and has been with CKLFHT 10+ years, newly supporting Fenelon Falls NP and Fenelon Falls lab.

Sarah Holroyde Nurse Practitioner
Graduated from the Primary Care Nurse Practitioner Program at Athabasca University. During her placements for the Primary Care NP degree, Sarah was responsible for managing the care of a variety of patients in both rural and inpatient hospital ward settings. Her placements included providing care to prenatal patients until time of delivery, pediatric patients, patients requiring comprehensive chronic disease management including mental health, as well as women’s health, opioid management, and palliative care. Sarah comes to us with a wealth of experience from over a decade of practicing emergency medicine as a Registered Nurse. She takes pride in her holistic, collaborative approach to patient care, and enjoys working with patients to determine the best care plan to suit their needs.
Laura Rea, Medical Receptionist
Laura is originally from Woodville and has been working as a Medical Secretary and Office Manager supporting an NP at VON in Kirkfield since 2012. Laura has also worked as a Medical Receptionist for VON Lindsay and Bethany, as well as an Employment Counsellor for the Victoria County Career Services. She has significant experience with PSS (as super user) as well as in creating custom forms, reports and running statistical data. Laura has also shown interest in social work by enrolling in few courses, has been trained in PHIPA, WSIB, UMAB (understanding and managing aggressive behaviour), and is certified in CPR.

Jennifer is a 2022 graduate of the MN-PHCNP Program from Queens University with some work experience as an NP in primary care, and RN in Neurology at UHN and Emergency Room at Michael Garron in Toronto. Jennifer and her family have recently relocated to Lindsay.

Michelle Sanderson RN BScN MN NP
Michelle Joined our team in 2022 with over 23 years of nursing experience. She completed her diploma of Nursing at Durham College and her Obstetrical Nursing at Humber College. Her under grad Nursing Degree is from Ryerson University in Toronto. Masters of Nursing was completed from Charles Sturt in Australia and her Nurse Practitioner from Athabasca in Alberta. Her career focus has been in Women’s Health and well babies. She also possesses many years of surgical and medical experience along with teaching experience.
Laura Devine RN BScN IBCLC ,
Started her career as a Registered Nurse working at Peterborough Regional Health Centre as a Paediatric Nurse. At her time in the hospital, she worked with surgical, gynaecology/obstetrics and worked in the NICU unit as well.
After working in the hospital, Laura taught Paediatrics/Obstetrics clinical to Registered Nursing Students at Sir Sanford Fleming College.
Later moved on to work in a Community Action Program for Children (CAPC), which is a Health Canada program at the Peterborough Family Resource Centre. Laura was team lead, working with at risk families with children from birth to 18 months. She contributed to other programs as well, facilitating and providing education on health, parenting, infant nutrition and healthy child development. During this time at this organization, she developed and opened a Well Baby Clinic, which aided parents with Breastfeeding and Healthy Child development (remains running today). Laura worked in partnerships with Healthy Baby Healthy Children’s program, Infant Development Program, Five Counties Children Centre, Kawartha Haliburton Children’s Aid Society and Ontario Works. She worked in the Learning, Earning and Parenting Program teaching teen parents. Topics that were taught were on self-esteem, health, parenting, and healthy child development. Laura earned the credential of Certified Community Health Nurse through the Canadian Nurses Association.
Laura went back to school to finish her Bachelor of Science in Nursing and graduated in 2014. While in school, she worked as a Supervisor for CanCare Community Health. In her role, she collaborated with Community Care Assessment Centre (CCAC) to assist patients discharged from hospital in providing service of care. She provided direction and support to nursing staff on her shift as well as client support.
Laura started with the FHT in 2010 for a brief time as a Family Practice Nurse. During the first year on the FHT she earned an International Board Certified Lactation Consultant certification. Laura restarted a Breastfeeding Coalition within the City of Kawartha Lakes with various community partners.
Happily, in 2014 Laura was able to return to her position as Family Practice Nurse with the City of Kawartha Lakes Family Health Team. As well as practicing as a Nurse/IBCLC she started a Breastfeeding Clinic with the Health Team in 2017.

Kelsie has been an NP at the North Durham FHT since 2017, which is also when she obtained her Primary Health Care NP license. Prior to her NP role at NDFHT, Kelsie practiced as a community health nurse for 7 years in the Northwest Territories, an ER Nurse and a clinical instructor in Nova Scotia, and a general duty nurse at the Lakes District Hospital in BC.

Della Hare, RN (EC) CDE
Della joins our Family Health Team having held various titles during her nursing career at the Ross Memorial Hospital. It was during her time at RMH that Della completed during her Masters in Nursing, leading her to further her career as a Nurse Practitioner in Primary Health Care.
In addition to being a licensed Nurse Practitioner, Della is also a Certified Diabetes Educator and brings with her a wealth of experience in general medicine, surgery, and critical care emergency nursing.

Karen joins our team with over 30 years’ experience as a medical secretary, and operated a foot care
department under the direction of four family physicians in Fenelon Falls. She is currently working with NP Della Hare in suite 2/3 at 55 Angeline Street N. in Lindsay, and continues to assist the Family Health Team After Hours Clinic. She obtained her CHRP in HR at Fleming College, and enjoys living and
promoting a healthy lifestyle. Karen enjoys running, golfing and being an active member in her
community of Fenelon Falls, where she resides with her family.

Maureen comes to us having completed her Bachelor of Science in Nursing at the University of Toronto as well as her Bachelor of Science in Human Kinetics at the University of Guelph. She has recently completed her Masters of Nursing through the University of Toronto. Maureen brings with her more than 11 years of experience having previously worked in the Emergency Department at the Peterborough Regional Health Centre.

Lori comes to our team with almost a decade of experience as a medical receptionist in primary care working with a team of three family physicians at Town and Country Medical in Lindsay. Prior to joining Town and Country Medical, Lori was an Administrative Assistant at a physiotherapy practice in Lindsay for several years. Lori holds a degree in environmental engineering and has been active in her local school board. A long-time resident of Little Britain, Lori brings her deep local knowledge – and friendly, caring disposition – to patients in Little Britain.

Hassan is a recent grad but he brings over six years of RN experience working in both acute and long-term care settings in urban and rural parts of Northern Ontario. Most of his clinical experience as a student Nurse Practitioner was in a family health clinic, where he carried a full patient load, with primary responsibilities for chronic disease management, chronic pain, mental health and addictions, well-baby visits and vaccinations.

Jo Ann Newell, Medical Administrator
Jo Ann has been a medical receptionist for more than 35 years. She is also the manager of the Kawartha Lakes After Hours Clinic. Jo Ann holds a certificate in medical administration from Durham College and has a sterling reputation for being patient-centred, supportive and flexible while retaining her outgoing and upbeat demeanour. Jo Ann and her family live here locally.

Debbie has been our Family Health Programs Co-ordinator for the past 10 years
and holds a Bachelor of Science in Nursing Degree from Ryerson University.
She has held several different nursing roles throughout her 30 + year career mainly in
Critical Care Emergency Nursing as well as Administrative Co-ordinator of the Nursing
Division at Ross Memorial Hospital.
Debbie’s special interest is health promotion and disease prevention.

Beth Syvret, RPh, PharmD
Beth joined our FHT in 2012 and brings experience in hospital, community and geriatric pharmacy practice.
A lifelong learner, Beth holds a BSc Phm from the University of Toronto, a Pharm D from the University of Florida and a Certificate in Pharmacy Management of Oral Anticoagulation Therapy from the University of Waterloo. She works with patients and other team members to optimize medication management.

Clare Millington, RPh, PharmD
Clare grew up in Lindsay and joined the Family Health Team in 2022. She has Bachelor of Science and Doctor of Pharmacy degrees from the University of Waterloo. Clare has experience working in community and hospital pharmacy settings. As one of the pharmacists at the FHT, she works closely with patients, physicians, and the interprofessional team to provide consultations and education about medications. Clare is dedicated to ensuring that individuals are receiving the most appropriate therapies for their health conditions, and to identifying, preventing, and resolving drug-related problems.

Tamara is new to the Family Health Team in 2022. As a resident of the City of Kawartha Lakes she comes to us as an RPN, with certification as a basic and advanced diabetic foot care specialist. Tamara brings to our team a variety of experience in the areas of community nursing, as well as surgical and stroke rehabilitation. Tamara’s ideology is that life is hard enough and with a health care system that is struggling she can make a difference……..ONE FOOT AT A TIME.

Grace is a dedicated Nutrition and Wellness Promotor with a passion for enhancing food literacy and promoting a balanced, enjoyable approach to nutrition. Having recently graduated with a Diploma in Dietetic Education & Practical Training, with a specialization in pediatrics, Grace brings a wealth of knowledge and expertise to the field.
Grace holds an Honours Specialization in Nutrition and Dietetics, building upon a solid foundation in biology. Additionally, she has a diploma in Culinary Management – Nutrition from George Brown College, showcasing a unique blend of culinary skills and nutritional understanding.
With a focus on promoting food enjoyment in moderation, Grace’s passion lies in helping others understand the beauty of food. She particularly specializes in diabetes management, using her expertise to guide individuals on the path to a healthier lifestyle. Grace believes in making nutrition education accessible and enjoyable, creating a positive impact on the well-being of others.

As a registered social worker, Susan started with the City of Kawartha Lake Family Health Team in December 2022. Susan has over 25 years of experience in the interdisciplinary social work system.
Susan has a Social Work Hons. Diploma from Humber College, Hons. B.A from Guelph University, and a Master of Social Work from Wilfrid Laurier University.
Susan has trained with many treatment modalities, including Cognitive Behavioural Therapy (levels 1 and 2), Solution Focused and Motivational Interviewing, Gottman Theory, Mindfulness, Acceptance Commitment Therapy (ACT) and many other certifications.
Susan values social justice and inclusivity and works with clients with a trauma-informed lens.
Susan likes to spend time in the outdoors to recharge.

Lisa Glover Carmichael, M.S.W., RSW
Having worked locally in the field of education for many years, Lisa comes to our team having conducted mental health assessment, treatment and service provision to adults and youth in community and hospital outpatient mental health settings in Haliburton, Minden, Cobourg and Lindsay. She has worked with children, youth and families, and with women of domestic violence. Lisa enjoys hiking and camping with her friends and family, and takes every opportunity to reset in nature.

Bernadette Jolicoeur, M.Ed., M.S.W., R.S.W.
Bernadette started with the City of Kawartha Lakes Family Health Team in June 2022 as a registered social worker. She has 25+ years of clinical social work experience working with children and youth at risk, adults, families, and groups in children’s mental health and hospital setting.
Bernadette has a Hons. B.A. from York University, a Master of Education (M.Ed.) from Ontario Institute of Studies in Education (OISE), and a Master of Social Work (M.S.W.) from University of Toronto. Bernadette has two-year psychotherapy training at Toronto Institute of Human Relation (TIHR).
Bernadette has trained with many treatment modalities, including Dialectic Behaviour Therapy (DBT), Unified Protocol (UP), Emotion Focus Therapy (level 1 and 2), Solution Focused and Brief Therapy, Collaborate Problem Solving, Interpersonal Relations, along with many of experiential study of mindfulness and meditation.
Bernadette colleagues describe her as hard-working, dedicated, passionate, courageous, friendly, and easy-to-talk-to. At home Bernadette loves to cook, bake, walk the dog, and play with her grandchildren. Bernadette has strong loving family connections.
Bernadette states at her heartfelt hope is that one day we will all work together to eradicate violence in society.
Clinical Social Worker
Lee-Ann Eastman MSW, RSW
Lee-Ann Eastman is a Registered Social Worker and a graduate from Lakehead University with a Master’s Degree in Social Work. Lee-Ann started with the City of Kawartha Lakes Family Health Team in November, 2022 and has 14 years of mental health experience in community mental health and hospital settings. Lee-Ann has extensive experience supporting people with trauma and specializes in grief, loss, depression, suicidal thoughts, anxiety and mood disorders.
Lee-Ann is trained in Dialectical Behavioral Therapy (DBT), Cognitive Behavioural Therapy (CBT), Compassion Focused Therapy (CFT),Solution Focused Therapy and Narrative Therapy and Mindfulness.
Lee-Ann’s expertise and training in a number of areas allows her to adopt her approach to each person’s unique needs. Lee-Ann enjoys yoga and spending time with family and friends.

Laura Drummond, BSW, RSW, MSW Candidate
Laura joined the City of Kawartha Lakes Family Health Team as a registered social worker in June 2024. She has worked as a social worker for over 12 years in community-based addiction, mental health, and justice system settings.
Laura has a Hons. B.A. in Social Development Studies and a Bachelor of Social Work (BSW), both from the University of Waterloo. After over a decade in the field, Laura has returned to school to complete a health-focused Master of Social Work (MSW) degree, finishing in August 2024.
Laura describes her approach to care as holistic, strengths-based, anti-oppressive, and trauma-informed. She has training in many modalities including Cognitive Behavioural Therapy, Motivational Interviewing, Community Reinforcement Approach, and Polyvagal Informed Treatment.
In her free time, Laura enjoys playing hockey, spending time with family and friends, camping, and cottaging.

Elizabeth Junkin-Fagan, RN
Elizabeth has been practicing family nursing since 2011 in collaboration with doctors in Lindsay. Providing primary care to all ages, she has a special interest in elderly care. A registered nurse since 2004, Elizabeth brings to our team a variety of experience including in the areas of community nursing and long term care. Elizabeth enjoys spending time with her family and 3 children.

Ashleigh Wilson, RN, B.Sc.N.
Ashleigh completed her nursing degree through York University and comes to our team with significant previous experience in cardiac intensive care. She is also knowledgeable in the areas of diabetes care and a variety of heart conditions. Ashleigh is especially interested in the quality of health care at the community level.

Carole has joined the FHT with 30 year’s experience as a Registered Nurse, 15 years in acute care at
PRHC and more recently 15 years at RMH in the Cardiovascular and Pulmonary Rehabilitation program
as a Certified Cardiovascular Nurse.
While at RMH she also developed and co-ordinated a Heart Failure Out -patient Clinic with the
assistance of physician specialists. Her passion remains in cardiovascular health promotion and
prevention.

Cherisse Young, RN
Cherisse graduated from Trent University with her BScN in 2008. From there Cherisse worked at
the Ross Memorial Hospital in the Emergency Department from 2008 to 2017. During this time,
studying to complete her Masters in Nursing and Primary Care Nurse Practitioner, which she
graduated in 2018. Cherisse has causally worked as a Nurse Practitioner since graduating and is
also a Professor of the Nursing Program at Fleming College, while working with the Family
Health Team as a part time Registered Nurse.

Aasif Khakoo, Executive Director
Aasif has over 25 years of experience working within interdisciplinary and multisectoral healthcare environments. He has extensive knowledge of the healthcare system, with strong professional background in strategic planning, human and financial resources management, as well as clinical programming. He holds strong values of social justice, equity and inclusiveness and would describe his leadership approach as patient-centric, authentic and solutions-oriented, with an emphasis on meaningful stakeholder collaboration and community partnerships.
Jean Walsh, Facilities Manager
Known as “the keeper of knowledge” Jean is responsible for ensuring the many aspects of our team run smoothly, including day-to-day operations, administrative issues and assisting staff at our team’s eight locations in the community. Living in Lindsay, Jean has been an executive assistant both for the Chief of Police of the City of Kawartha Lakes and in the senior ranks of the private sector. Jean has also worked as Logistics Coordinator, Document Controller and Senior Operations Coordinator with national and international companies.
Kim Smith, CHRP HR Manager
A certified human resources leader (CHRL), Kim joined our team in 2013 having previously provided 14+ years of administrative, financial, human resource and accreditation support to the Canadian Mental Health Association Haliburton Kawartha Pine Ridge Branch (CMHAHKPR). Kim completed her HR Management certification with Sir Sandford Fleming College and is a certified member of the Human Resources Professional Association (HRPA). Kim enjoys rural living in the Kawarthas and loves to travel with her family whenever possible!
Krissy Hickson, Finance Manager
Krissy holds an honours diploma in accounting and payroll administration from Trillium College and has a wealth of financial management experience. When not doing FHT finances, Krissy is the Housing Manager with several non-profit organizations that house individuals that suffer from the effects of mental illness. In addition to her expertise, Krissy brings with her an energetic, friendly personality that will ensure she is a great match with our team
Danielle VanGennip, Manager of Operations
Danielle brings her excellent administrative, management, and people skills and experience within our FHT. Before coming to the FHT, Danielle worked as a district manager at a popular children’s store. Danielle has a background as a medical receptionist as well as an IT ‘superuser’. A graduate of Fleming College as a Personal Support Worker, Danielle has lived in our area her entire life. Danielle takes pride in helping the residents of Kawartha Lakes.
