Diana Duclos-King, MN, RN (EC)
Diana is a recent addition to the CKLFHT in Bobcaygeon, taking over Linda Ready’s practice. Originally from Quebec, she has a DEC (Diplôme d’études collégiales) in Nursing from Dawson College, a BScN from St. Francis Xavier University and a Master of Nursing in Nurse Practitioner from Athabasca University. Her nursing background is emergency/critical care. Diana has a proactive and hands-on approach to patient-centered care and looks forward to getting to know the area.

Jackie has a decade of experience working in multiple hospital settings. She is a native to Reaboro and currently lives in Ennismore. Jackie holds a diploma from St. Lawrence College in Kingston in Medical Laboratory Assistant as well as Health Information Management. Jackie currently provides administrative support to our Nurse Practitioner at our Bobcaygeon office.

Natalie Westwood, RN (EC)
Natalie holds a Masters in Nursing (Primary Care) from York University and received her B.Sc. in nursing from Trent. Natalie has had experience in a variety of clinical settings including with the Algonquin South Family Health Team, emergency care at Mt. Siani Hospital, and internal medicine nursing at St. Mike’s. Natalie’s interests include palliative care and she has expressed a passion for rural health. Living in Oshawa, Natalie is looking forward to practicing in our area.

Kylie practices family nursing at the Fenelon Medical Centre, having worked previously in the surgical unit at the hospital in Peterborough. Kylie has also worked at Pinecrest Nursing Home in Bobcaygeon and has clinical experience with children and new moms. Kylie holds a degree in community health from Brock and a nursing science from Trent University. She grew up in Fenelon and now lives in Bobcaygeon.

Cheryl Marks,NP-PHC, MN, CIC
Cheryl joins us from the North Huron Family Health Team in Wingham, Ontario. Previously, she served as Director of Nursing at a long-term care facility in Bancroft. Cheryl’s nursing career focused on neonatal intensive care, infectious disease control, and sexual assault treatment. Cheryl holds a Master of Nursing-Primary Care from Athabasca University and a Bachelor of Science degree from Lakehead University. She and her husband are moving to our area to live just east of Lindsay. Cheryl’s hands-on approach and flexible, caring disposition will make her an asset to our team.

Dana Robertson, Medical Receptionist
Dana has been the medical receptionist at Woodville since last November, working for Community Care of Kawartha Lakes. Now with our FHT team, Dana will provide medical reception service to Woodville patients, ensuring continuity of care and the comfort of a familiar face to patients, given the recent changes in providers. Dana is highly committed to her work and enjoys helping people, so will fit in very well with our FHT. Originally from Pontypool, Dana lives in Lindsay.

Laura Devine, BScN, RN, IBCLC
Laura holds a Bachelor of Science degree in Nursing from Ryerson University, is a Certified Lactation Consultant and former Certified Community Health Nurse. Laura has a strong nursing background in Pediatrics and Maternity with an emphasis on healthy child development. She has taught clinical at the college level in the Registered Nurse program.

Della Hare, RN (EC) CDE
Della joins our Family Health Team having held various titles during her nursing career at the Ross Memorial Hospital.  It was during her time at RMH that Della completed during her Masters in Nursing, leading her to further her career as a Nurse Practitioner in Primary Health Care.

In addition to being a licensed Nurse Practitioner, Della is also a Certified Diabetes Educator and brings with her a wealth of experience in general medicine, surgery, and critical care emergency nursing.

Maureen comes to us having completed her Bachelor of Science in Nursing at the University of Toronto as well as her Bachelor of Science in Human Kinetics at the University of Guelph. She has recently completed her Masters of Nursing through the University of Toronto.  Maureen brings with her more than 11 years of experience having previously worked in the Emergency Department at the Peterborough Regional Health Centre.

Lori comes to our team with almost a decade of experience as a medical receptionist in primary care working with a team of three family physicians at Town and Country Medical in Lindsay. Prior to joining Town and Country Medical, Lori was an Administrative Assistant at a physiotherapy practice in Lindsay for several years. Lori holds a degree in environmental engineering and has been active in her local school board. A long-time resident of Little Britain, Lori brings her deep local knowledge – and friendly, caring disposition – to patients in Little Britain.

Summer works at our Team’s location at the Omemee Medical Centre, where she arranges appointments and patient care with our Team’s nurse practitioner there. Summer is a long-time resident of the Omemee area, with medical office experience and a diploma in medical terminology from Sir Sandford Fleming College.

Jo Ann has been a medical receptionist for more than 35 years. She is also the manager of the Kawartha Lakes After Hours Clinic. Jo Ann holds a certificate in medical administration from Durham College and has a sterling reputation for being patient-centred, supportive and flexible while retaining her outgoing and upbeat demeanour. Jo Ann and her family live here locally.

Debbie McLeod, B.Sc.N, ENCc, RN
Debbie is the coordinator of our team’s many family health programs. Recently completing her nursing degree where she focused on community health programs, Debbie has many years of experience at the Ross Memorial Hospital including as team leader in the emergency department and administrative coordinator of the nursing division. Debbie’s special interest is health promotion. She and her family live in Lindsay.

Tanya Nye, M.Sc. O.T., O.T. Reg. (ON)
As our team’s occupational therapist, Tanya sees patients to help them maximize their health within their environments, include through assessing home and work safety, increasing mobility and promoting injury prevention. Tanya also works with seniors to remain in their homes for as long as they are able. Tanya has focused her career working in the community and has experience working in the auto sector and home care, assisting people of all ages return to their daily activities. Tanya is from Dunsford and comes to our team with a Master’s degree in occupational therapy from Queen’s University and an undergraduate degree in human kinetics.

Beth comes to our team from Peterborough where she most recently held a position of pharmacy manager. She has worked as a pharmacist at the Ross Memorial Hospital and as a clinical pharmacist with the GAIN Geriatric Clinic. Beth also brings a decade of experience working in retail/community pharmacy to her work with our local physicians and patients.

Wendy comes to our team with a background in medical and geriatric nursing. Wendy holds a certificate in advanced foot care nursing. Wendy is passionate about chronic disease management and advocates that foot care is necessary to prevent illness and maintain overall good health.

Cindy Billiaert, RRT, CRE
Cindy sees patients for education on respiratory disease processes, drug and inhaler therapy, self-management and environmental controls. A graduate of Algonquin College, Cindy comes to our team with experience at several Toronto hospitals, including The Hospital for Sick Children. Cindy is active in asthma/COPD education and pulmonary function testing. She lives near Manilla.

Alexandra Burghardt, Registered Dietitian
As one of our team’s Registered Dietitians, Alex provides evidenced-based medical nutrition therapy and helps individuals to assess their nutritional status and work towards their nutrition-related goals. Alex holds a Bachelors of Applied Science in Nutrition and Food from Ryerson University and completed her Professional Master’s Diploma in Dietetics and practical dietetics training through Ryerson University and St. Michael’s Hospital in Toronto. Alex comes to our team with experience in wide-range of healthcare settings including primary care, diabetes education, and inpatient/hospital care.

Sarah Burnett, Registered Dietitian
Sarah is a Registered Dietitian who is passionate about and skilled in promoting health behaviour change to help individuals meet their nutrition and health goals. She holds a Masters of Public Health in Nutrition and Dietetics from the University of Toronto where she completed clinical placements in outpatient cardiac care at Toronto Western Hospital and in a community health centre. She previously worked in public health where she managed nutrition programs and has extensive experience planning and delivering group education sessions for wide-ranging health conditions and age groups.

Aziel holds an M.S.W. from Lakehead University and has more than 10 years of experience with Kinark locally, working mainly with children and families conducting assessments, providing counselling, co-creating treatment plans and managing therapeutic relationships. She has also done counselling with older adults at Riverview Manor. Aziel’s friendly disposition and flexible approach make her a great fit with our team.

Having worked locally in the field of education for many years, Lisa comes to our team having conducted mental health assessment, treatment and service provision to adults and youth in community and hospital outpatient mental health settings in Haliburton, Minden, Cobourg and Lindsay. She has worked with children, youth and families, and with women of domestic violence. Lisa lives with her family in the Lindsay area.

Riley is our new Clinical Social Worker filling in for Lisa Ann. Riley is a Registered Social Worker and has a Master of Social Work from Lakehead University which she completed following her Bachelor of Social Work at Lakehead University and Bachelor of Arts in Psychology at Wilfrid Laurier University. With experience supporting individuals and families in a broad range of concerns, Riley enjoys incorporating empathy, compassion, and humour into her clinical work. She believes in providing individuals with information, tools, and resources to guide their autonomous mental wellness. Riley is an animal lover and enjoys time outside, baking, and volunteering when not at work.

Elizabeth Junkin-Fagan, RN
Elizabeth practices family nursing in collaboration with doctors in Lindsay. A registered nurse since 2004, Elizabeth brings to our team a variety of experience including in the areas of community nursing, long-term care, and geriatric medicine/rehabilitation therapy both in our area and in British Columbia. Elizabeth also has a depth of knowledge in wound care and IV therapy.

Ashleigh completed her nursing degree through York University and comes to our team with significant previous experience in cardiac intensive care. Lesley practices family nursing in Lindsay, having previously worked extensively in the emergency departments at both the Peterborough and Haliburton hospitals. With a degree in nursing science from Queen’s University, Lesley is certified in coronary care and has a specific interest in child birth care and community health promotion. She has also served in a teaching role in nursing. Lesley is originally from Haliburton.

Aasif Khakoo, Executive Director

Aasif has over 25 years of experience working within interdisciplinary and multisectoral healthcare environments. He has extensive knowledge of the healthcare system, with strong professional background in strategic planning, human and financial resources management, as well as clinical programming. He holds strong values of social justice, equity and inclusiveness and would describe his leadership approach as patient-centric, authentic and solutions-oriented, with an emphasis on meaningful stakeholder collaboration and community partnerships.

Jean Walsh, Operations Manager
Known as “the keeper of knowledge” Jean is responsible for ensuring the many aspects of our team run smoothly, including day-to-day operations, administrative issues and assisting staff at our team’s eight locations in the community. Living in Lindsay, Jean has been an executive assistant both for the Chief of Police of the City of Kawartha Lakes and in the senior ranks of the private sector. Jean has also worked as Logistics Coordinator, Document Controller and Senior Operations Coordinator with national and international companies.

Kim Smith, CHRP HR Manager
A certified human resources professional, Kim joins our team following many years of human resources experience with the Canadian Mental Health Association Kawartha Lakes Branch. As Human Resources Manager, Kim was involved in all aspects of human resources management including staff supervision, coordinating workplace health and safety and providing leadership support for workplace wellness. She was also a leader in the organization’s accreditation process. Kim has additional experience in financial administration. Kim and her family live in Fenelon Falls.

Krissy Hickson, Finance Manager
Krissy holds an honours diploma in accounting and payroll administration from Trillium College and has a wealth of financial management experience primarily in the private sector and with the Canadian Mental Health Association, Kawartha Lakes Branch.  When not doing FHT finances, Krissy is the Executive Director of the Fenelon & Area Independent Living Association in Fenelon Falls.  In addition to her expertise, Krissy brings with her an energetic, friendly personality that will ensure she is a great match with our team.

Danielle VanGennip, Executive Assisant
Danielle brings her excellent administrative, management, and people skills and experience within our FHT. Before coming to the FHT, Danielle worked as a district manager at a popular children’s store. Danielle has a background as a medical receptionist as well as an IT ‘superuser’. A graduate of Fleming College as a Personal Support Worker, Danielle has lived in our area her entire life. Danielle takes pride in helping the residents of Kawartha Lakes.
